Jewish Family Home Care- a not-for-profit agency – is proud to partner with MedBetterHealth, a Centers for Medicare & Medicaid Services (CMS) GUIDE Model participant, to expand access to comprehensive, home-based dementia care for patients and caregivers across South Florida.
This collaboration brings together MedBetterHealth's CMS-recognized care coordination model and Jewish Family Home Care's award-winning services, offering enhanced dementia support, respite care, and caregiver education. The partnership will allow Jewish Family Home Care to provide high-quality dementia care through the GUIDE Model while continuing to serve seniors with the compassion and dignity for which the agency is known. The program includes 77 hours of care valued at over $2,500.
Jewish Family Home Care provides multi-reach, non-medical, quality home health care for Broward County, Florida, seniors from all reaches of life and any faith.Jewish Family Home Care is oneof the largest providers ofhome health care, providing assistanceto Holocaust survivors and anyone in Broward County needing in-home care. Providing servicetomore than 400 clients, including over 300 Holocaust survivors, Jewish Family Home Care offers services such as home health aides, companions, housekeepers and certified nursing assistants.
