NEW YORK, NY / ACCESS Newswire / February 24, 2025 / Heartland Community Health Centerrecentlydisclosed that it suffered a data breach that compromised the sensitive personal and protected health data of individuals. This data breach has led to concerns over the security of sensitive personal and protected health information entrusted to Heartland.
WHAT’S THIS ABOUT?
On October 1, 2024, Heartland became aware of a security incident involving an employee’s email account. Upon detection, Heartland implemented security measures, including resetting employee passwords, and engaged third-party cybersecurity experts to conduct a thorough investigation. The investigation determined that an unauthorized third party accessed certain information within the email account between October 1, 2024, and October 2, 2024. Heartland then conducted a thorough review of the affected data to determine what information may have been compromised. On January 10, 2025, Heartland completed its review.
Upon information and belief, the following types of sensitive personal and protected health information may have been compromised: name, Social Security number, address, date of birth, contact information (e.g., phone number, email address), driver’s license/state identification number, medical information (e.g., medical diagnosis/treatment information, prescription information, date of service, patient ID number, provider name, medical record number), medicare/medicaid number, and health insurance information (e.g., health insurance claim number, health insurance policy number, treatment cost information). Heartland posted notice of this incident on its website on November 27, 2024, and began mailing written notices to affected individuals on February 10, 2025. Compensation may be available for those individuals who received notice that their personal information was compromised.
