On or about January 28, 2025, Persante Health Care became aware of unusual activity on its network. Upon discovery, Persante Health Care launched an investigation with the assistance of external cybersecurity experts to determine the nature and scope of the incident. The investigation revealed that an unknown actor gained access to the network between January 23, 2025, and January 28, 2025, and potentially accessed or acquired files containing sensitive personal and protected health information.
The impacted data may include names, dates of birth, Social Security numbers, driver’s license numbers, state identification numbers, passport numbers, governmental identification numbers, individual taxpayer identification numbers, dates of medical service, physician or medical facility information, medical diagnosis or treatment information, medicare or medicaid numbers, individual health insurance policy numbers, financial account numbers, payment card numbers, patient account numbers, medical record numbers, medical device identifiers, and biometric identifiers.
On November 26, 2025, Persante Health Care publicly disclosed the data breach on its website and began mailing notice letters to affected individuals. Compensation may be available for those individuals who receive notice that their sensitive personal information was compromised.
